Licensed Occupational Therapist (OT) - Cherrelyn Healthcare Center | Littleton, CO

Part-Time & Full-Time | $45/hr - $52/hr DOE
What You're Walking Into

Cherrelyn Healthcare Center is a 176-bed skilled nursing and rehabilitation facility in Littleton, CO, part of Stellar Senior Living and operating for 12+ years. You'll join a tenured interdisciplinary therapy team in a stable, resident-centered SNF environment where OTs actually have the time and autonomy to do quality clinical work.

This is not a high-volume, high-turnover setting. It's a place where you can build a real caseload, supervise COTAs well, and see meaningful outcomes with your residents.
The Role

As the supervising OT, you'll evaluate new admissions, develop individualized treatment plans, and lead OT services focused on ADL retraining, cognitive-functional rehab, and return-to-home planning. You'll also provide clinical direction to COTA staff on your cases.

Day-to-day responsibilities:
  • Conduct initial evaluations and establish measurable, resident-centered treatment goals
  • Develop and implement individualized OT plans addressing ADLs, IADLs, cognition, fine motor function, and safety
  • Supervise and collaborate with COTA staff to ensure plan-of-care fidelity
  • Recommend and train residents on adaptive techniques and assistive devices
  • Partner with PT, SLP, nursing, and social services on interdisciplinary care planning
  • Educate residents and families on goals, progress, home strategies, and discharge planning
  • Complete, accurate, timely documentation in compliance with Medicare and state standards
  • Monitor resident progress and modify treatment plans accordingly

Requirements
  • Active Colorado OT license in good standing
  • Degree from an accredited Occupational Therapy program (MOT or OTD preferred)
  • SNF or post-acute experience preferred - strong new graduates with fieldwork in SNF settings will be considered
  • Experience supervising COTAs is a plus
  • Strong clinical reasoning, documentation, and communication skills
