Salary Grade: 23Salary Range: $86,502 - $100,904 The Research Foundation for Mental Hygiene, Inc. is seeking a qualified candidate to fill the position of Legal Partnership Case Manager. The Medical/Public Health-Legal Partnership Case Manager serves as a liaison between program participants, healthcare/public health teams, and external legal partners.
This role identifies and addresses legal needs that impact health outcomes, ensuring participants receive coordinated legal support. The Case Manager conducts intake and assessment, runs conflict of interest checks, develops service plans, facilitates referrals, and monitors participant progress toward defined legal and health-related goals.
Key Responsibilities:- Receive and review referrals from program staff regarding participants' legal needs.
- Conduct comprehensive intake, screening, and assessment to identify legal and social determinants impacting health.
- Run conflict of interest checks with the potential legal partner the participant will be paired with
- Develop individualized service plans in collaboration with participants, healthcare providers, and legal partners.
- Serve as a bridge between participants, program staff, and external legal service providers to ensure effective communication and coordination.
- Facilitate referrals to appropriate legal resources and support participants through the referral and engagement process.
- Follow up on active cases to ensure continuity of care and progress toward resolution of legal issues.
- Manage and prioritize a caseload, ensuring timely and effective service delivery.
- Monitor participant progress toward legal partnership goals and adjust service plans as needed.
- Maintain accurate, timely, and confidential documentation of all case activities, outcomes, and communications.
- Track and report program metrics, outcomes, and trends related to legal needs and interventions.
- Participate in interdisciplinary team meetings and contribute to program development and quality improvement efforts.
- Educate participants and staff on available legal resources and the impact of legal issues on health outcomes.
Minimum Qualifications:- Bachelor's degree in social work, public health, human services, or a related field required
- 3 years of experience in case management, care coordination, or social services, preferably within healthcare, public health, or legal services settings.
Preferred Qualifications:- Master's degree preferred
- Experience working within a Medical-Legal Partnership (MLP) or similar integrated service model.
- Experience working with diverse, criminal legal system-impacted, and underserved populations.
- Strong organizational skills with the ability to manage multiple cases and priorities effectively.
- Excellent communication and interpersonal skills, with the ability to collaborate across multidisciplinary teams.
Work Environment:- This position may operate in healthcare, public health, or community-based settings and may involve a combination of in-person and remote work.
- Occasional local travel may be required to meet with partners or participants.
