Job Summary:

The Justice-Involved Site Manager provides strategic leadership and day-to-day oversight of Serene Health's Enhanced Care Management (ECM) services for individuals transitioning from incarceration within a single office location. This role directs Lead Care Managers, strengthens custody-to-community care transitions, ensures consistent use of internal policies and workflows, makes data-informed operational decisions, and builds effective partnerships with Medi-Cal managed care plans, correctional facilities, county agencies, health care providers, and community-based organizations.
Duties/Responsibilities:
  • Oversee and manage all Justice-Involved staff members across all office locations.
  • Direct, organize, and continuously improve justice-involved ECM operations across multiple offices.
  • Establish and monitor staffing patterns, program goals, workflows, and performance objectives aligned with Serene Health requirements and applicable contracts.
  • Lead regular team meetings to communicate priorities, resolve operational barriers, review member-transition needs, and sustain a high-performance culture.
  • Maintain current working knowledge of ECM and Justice-Involved Reentry Initiative guidance and translate relevant changes into operational practice.
  • Champion consistent application of internal policies, workflows, escalation pathways, and documentation standards; identify gaps and lead corrective action.
  • Partner with Human Resources on recruitment by interviewing candidates and making hiring recommendations for open positions.
  • Manage onboarding, orientation, and role-specific training for staff serving justice-involved members.
  • Provide regular coaching, case consultation, and structured supervision that reinforces accountability, cultural humility, trauma-informed practice, staff safety, and professional growth.
  • Conduct performance reviews, set clear goals, document outcomes, and implement progressive disciplinary action in accordance with organizational policy.
  • Coordinate termination processes with Human Resources, ensuring procedural fairness, thorough documentation, and adherence to organizational guidelines.
  • Review, approve, and submit payroll accurately and on time; resolve discrepancies and follow applicable wage-and-hour requirements.
  • Oversee timely engagement of referred members before or after release, as applicable to the member's pathway and Serene Health's assigned scope.
  • Ensure care teams support individualized, person-centered transition planning with the member and coordinate warm handoffs from pre-release care management to community-based ECM when applicable.
  • Promote continuity of physical health, behavioral health, substance use disorder, pharmacy, dental, social, and other needed services during the transition to the community.
  • Ensure each enrolled member has an individualized care plan that is co-created with the member, updated as needs change, and actively monitored for progress and barriers.
  • Monitor outreach, engagement, enrollment, care-plan completion, appointment connection, referral follow-through, and transition outcomes; intervene when performance or member safety concerns arise.
  • Ensure staff use respectful, non-stigmatizing, strengths-based, culturally responsive, and trauma-informed approaches when serving adults and youth affected by incarceration.
  • Oversee quality review, case escalation, incident reporting, and corrective-action processes within the manager's authority and in coordination with ECM Leadership.
  • Serve as the primary site-level liaison among Serene Health ECM, Medi-Cal managed care plans, correctional facilities, county agencies, probation or parole partners when involved in the member's care, health care providers, and community-based organizations.
  • Develop practical referral, information-exchange, release-notification, appointment-linkage, and escalation workflows with partners while protecting member privacy and following applicable organizational requirements.
  • Coordinate resolution of barriers involving member identification, referral status, release timing, contact information, benefit access, transportation, medications, durable medical equipment, housing, and other transition needs within the team's scope.
  • Represent Serene Health professionally in partner meetings, implementation workgroups, network convenings, and community collaborative forums.
  • Build and maintain a reliable network of reentry resources and facilitate closed-loop referrals without promising services or eligibility that have not been confirmed.
  • Ensure accurate, timely, and complete documentation in the designated electronic health record and ancillary systems to support quality care, program oversight, compliance, and reimbursement.
  • Extract, analyze, and synthesize productivity, engagement, quality, and transition data to generate actionable management reports.
  • Use caseload metrics, referral trends, release timelines, outreach results, care-plan status, and outcome indicators to set priorities, allocate resources, and identify improvement opportunities.
  • Establish and track key performance indicators for team productivity, member engagement, warm handoffs, documentation timeliness, care-plan completion, referral closure, and program compliance; present findings to leadership.
  • Protect the confidentiality and integrity of member information and ensure staff follow approved data-sharing and record-management processes.
  • Performs other duties as assigned.
Education and Experience:
  • Bachelor's degree from an accredited college or university required; an equivalent combination of education and relevant experience may be considered.
  • Minimum of seven years of progressive experience in a health care, care management, behavioral health, social services, reentry, or closely related setting.
  • Demonstrated experience managing or supervising multidisciplinary care teams.
  • Demonstrated professional experience serving justice-involved adults and/or youth and coordinating reentry-related health or social services.
  • Experience collaborating with correctional facilities, Medi-Cal managed care plans, county agencies, behavioral health systems, and community-based organizations strongly preferred.
Required Skills/Abilities:
  • Working knowledge of social determinants of health and the barriers that can affect health, engagement, and stability during reentry.
  • Ability to lead person-centered, trauma-informed, culturally responsive, recovery-oriented, and non-stigmatizing services.
  • Demonstrated cultural humility and ability to work effectively with diverse populations and communities.
  • Strong knowledge of care coordination, transition planning, multidisciplinary case review, closed-loop referral practices, quality improvement, and operational compliance.
  • Bilingual in English and Spanish preferred.
  • Strong proficiency in Microsoft Office Suite and internet-based applications.
  • Skill in navigating multiple electronic health record systems while maintaining data integrity and accuracy.
  • Excellent verbal, written, and interpersonal communication skills, including the ability to engage staff, members, executive leaders, health plan representatives, correctional partners, and community stakeholders.
  • Strong analytical and data-informed decision-making skills, with the ability to interpret complex information, identify trends, and translate findings into measurable operational improvements.
  • Ability to implement and continuously refine internal policies and workflows while supporting consistent adoption across the site team.
  • Exceptional judgment, organization, and prioritization skills in a fast-paced environment with time-sensitive member transitions.
Physical Requirements:
  • Willingness and ability to travel to Serene Health offices and partner locations for oversight, training, meetings, and care-transition coordination.
  • Ability to follow site-specific safety, security, identification, and clearance procedures when entering partner or correctional settings, where applicable.
  • Ability to remain seated at a desk and work at a computer for extended periods.
  • Ability to lift up to 10 pounds occasionally.


Pay range

$76,000-$105,000 USD
