Full Job Description
Position Overview
United Health Services (UHS) is seeking a dedicated Pharmacist to join our Transitions of Care Pharmacy team. The Pharmacist will ensure the safe and accurate administration of medications by verifying the clinical appropriateness of prescribed treatments and overseeing proper delivery methods, thereby promoting patient safety and optimizing therapeutic outcomes. The Pharmacist is responsible for reviewing complex medication regimens, including prior to admission medications and discharge medications. The Pharmacist will work to ensure a smooth transition from hospital to home, including addressing adherence barriers such as medication affordability and transportation barriers. The pharmacist will also provide patient/caregiver education on medications and pertinent disease states.

Compensation Range:
$54.17 - $81.25 per hour, depending on experience

Job Responsibilities

  • Monitor patient therapy outcomes and adjust or recommend changes to optimize treatment
  • Provide drug information to healthcare providers, patients, and caregivers
  • Provide patient/caregiver education on medications and pertinent disease states
  • Evaluate and prevent adverse drug events and medication errors
  • Assess and address adherence barriers to medications prior to discharge
  • Document all activities in the electronic health record (Epic)
  • Ensure communication and coordination between healthcare providers and patient/caregivers
  • Ensure compliance with state and federal regulations

Position Qualifications

Minimum Required:
  • A Bachelor's of Science Degree (B.S.) or Pharm.D. in Pharmacy from an accredited college or university
  • Licensed as a Pharmacist in New York State or NYS internship permit

Preferred:
  • One year experience as a pharmacist
