JOB SUMMARY:

Sapphire Health Services is seeking an experienced and adaptable Interim Director of Health Services (DHS), RN to provide temporary clinical leadership and operational support across multiple Assisted Living, Memory Care, and Behavioral Health communities throughout Oregon and Washington.

This role is ideal for a highly skilled nurse leader who thrives in dynamic environments and can quickly assess needs, stabilize operations, support teams, and ensure regulatory compliance. The Interim DHS will step into communities requiring temporary leadership coverage, transition support, census growth support, survey readiness, staff mentoring, or operational improvement.

The ideal candidate is a strong clinical leader with experience in senior living who can build trust quickly, lead with compassion, and maintain high standards of resident care and compliance.

RESPONSIBILITIES:

  • Provide interim clinical leadership and oversight for Assisted Living, Memory Care, and/or Behavioral Health communities as assigned
  • Oversee resident care services to ensure quality outcomes, resident safety, and regulatory compliance
  • Ensure compliance with all applicable state regulations, licensing requirements, and company policies in Oregon and Washington
  • Assess community clinical operations and identify opportunities for process improvement, resident care enhancement, and operational stabilization
  • Supervise, mentor, and support nursing and caregiving teams including onboarding, coaching, accountability, and performance management
  • Support staffing initiatives, scheduling oversight, and development of care teams to maintain quality resident care
  • Conduct resident assessments, service planning, and care coordination as needed
  • Review and monitor resident documentation, medication management systems, physician orders, and incident reporting
  • Partner with Executive Directors and regional leadership to support community goals, survey readiness, occupancy growth, and quality initiatives
  • Assist with state surveys, audits, plans of correction, and follow-up compliance activities
  • Support transitions during leadership vacancies, leaves of absence, new acquisitions, or operational changes
  • Promote a positive team culture focused on resident-centered care, accountability, and collaboration
  • Travel to assigned communities throughout Oregon and Washington based on business needs
  • Other Duties as Assigned


REQUIREMENTS:

  • Active Oregon and/or RN License
  • Proven experience in Community Based Care settings
  • Proficiency in Oregon and/or Washington laws and regulations pertaining to healthcare
  • Strong leadership skills with the ability to motivate a diverse team
  • Excellent communications and interpersonal abilities
  • Commitment to providing compassionate and quality care to our residents
