As our Integrated Care Supervisor, you'll have the opportunity to:
• Lead and support a team of Care Coordinators, Targeted Case Managers, and Care Managers
• Promote integrated, person-centered, trauma-informed care across services
• Collaborate with medical, behavioral health, and community partners to improve outcomes
• Support care transitions and continuity of care across healthcare systems
• Mentor, coach, and develop employees through clinical leadership and supervision
• Drive quality improvement initiatives and ensure compliance with CCBHC standards

Annual Salary: $93,600.00

What You'll Do

As the Integrated Care Supervisor, you will provide clinical and operational oversight of integrated care services while supporting staff, individuals served, and organizational goals.

Your responsibilities will include:
  • Supervising and supporting Care Coordinators, Targeted Case Managers, and Care Managers
  • Providing clinical consultation and guidance for complex medical and behavioral health situations
  • Monitoring care coordination activities, documentation, and service delivery
  • Supporting transitions of care between hospitals, primary care, behavioral health, crisis services, and community resources
  • Partnering with healthcare providers to ensure coordinated, whole-person care
  • Identifying barriers to care and facilitating appropriate interventions and referrals
  • Monitoring quality metrics, outcomes, and performance indicators
  • Assisting with staff recruitment, onboarding, training, and professional development
  • Supporting compliance with federal, state, nursing, and CCBHC requirements
  • Developing and maintaining strong relationships with healthcare and community partners
  • Assisting leadership with program development, workflow improvements, and operational planning
  • Serving as a clinical resource to interdisciplinary teams throughout the organization

What We're Looking For

Required Qualifications
  • Current, valid Montana Registered Nurse (RN) license in good standing
  • Minimum of two years of supervisory experience
  • Strong clinical assessment, critical thinking, and care coordination skills
  • Excellent communication, leadership, and problem-solving abilities
  • Proficiency with electronic health records (EHR) and Microsoft Office applications
  • Valid MT driver's license, personal vehicle, and personal insurance
  • Ability to travel to all Many Rivers Whole Health locations as needed

Preferred Qualifications
  • Two years of nursing experience
  • Experience in Care Coordination and/or Case Management
  • Experience working in behavioral health, community health, or integrated healthcare settings
  • Knowledge of CCBHC standards and trauma-informed care practices
  • Familiarity with healthcare delivery systems and community resources
