Within the context of a client centred framework and continuum-based care model and in accordance with British Columbia College of Nurses and Midwives (BCCNM) for a Registered Nurse or a Registered Psychiatric Nurse, standards of professional practice, Code of Ethics, and the vision and values of the organization, works as a member of an interdisciplinary team to conduct the intake and transition functions for the designated site/program services. Provides clinical judgement and guidance for initial client screening, assesses referrals and clients based on client presentation, urgency of needs and best approach to determine the appropriate intake treatment. Coordinates discharge and transitions, as needed. Supports client transition by monitoring client connection to clinic and/or 3rd parties. Assesses and communicates clinic capacity to accept new referrals to health providers and the general public. Establishes and enhances relationships/communication within the designated program services and other community resources. Collaboratively develops transition plans for new referrals, initiates and facilitates interdisciplinary care conferences, prepares, and maintains client records, maintains and provides accurate intake/referral data and statistics, participates in team meetings and continuing education, and engages in on-going team planning and quality improvement.

About the Intensive Health Outreach Team (IHOT):
This is an exciting position with the Intensive Health Outreach Team! You will work as part of an interdisciplinary team to provide outreach services for individuals who are experiencing homelessness in shelters, on the street, and in encampments. You will provide episodic primary care, mental health and substance use supports to clients in the form of immediate assessment, intervention, treatment and bridging. Qualifications

Education & Experience

• Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
• Two (2) years recent related experience in the designated clinical program area or an equivalent combination of education, training and experience.

Knowledge, Skills & Abilities

• Broad knowledge of nursing theory and practice within a client and family centered care model.
• Broad knowledge of the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for nursing.
• Demonstrated ability to provide client-focused care in completing initial client assessments and interventions.
• General of knowledge Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.
• Demonstrated ability to promote Indigenous Cultural Safety.
• Demonstrated ability to provide competent and culturally safety care in a variety of settings and with diverse populations.
• Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care.
• Knowledge of community resources available for client/family support and related health services.
• Broad knowledge of other health care disciplines and their role in client care.
• Demonstrated ability to problem solve using sound judgement in applying critical thinking skills within safe limits of client care.
• Demonstrated ability to communicate effectively, both orally and in writing.
• Demonstrated listening and information seeking skills that promotes communication and lead to a co-operative approach to problem solving within a multidisciplinary setting.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
