Full Job Description
Inpatient cardiology pharmacist position at Mayo Clinic Hospital in Rochester, MN. In this role, the pharmacist provides exceptional care to patients undergoing cardiac procedures or receiving cardiology care in our cardiology intensive care unit or general cardiology units. Decentral clinical services consist of collaborative practice with multidisciplinary teams, identification of drug-related problems, anticoagulation management and stewardship, pharmacokinetic monitoring, provision and documentation of recommendations through progress notes regarding optimization of drug therapy, multidisciplinary team rounding, admission and discharge medication reconciliation, transition of care planning, and direct patient care. Job Responsibilities include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication monitoring and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Precepting of Mayo Clinic pharmacy and other disciplines' learners is expected. Involvement in scholarly activities including quality initiatives and multidisciplinary research is encouraged and supported through extensive institutional and departmental resources. The following are preferred: Previous experience including PGY1 plus 1 year experience in cardiology clinical practice, PGY2 training in cardiology or related clinical specialty, or equivalent experience of 3 years in cardiology clinical practice Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations Relevant applicable pharmacy experience working in a progressive and collaborative practice environment Experience and/or interest in teaching and/or precepting Board Certification in Pharmacotherapy Specialty (BCPS) License or certification: License to practice Pharmacy in the State of Minnesota or eligibility for licensure.
