Overview

Clinical Liaison VIPC, Inpatient Hospice Admissions RN

Location: Float RN

Position Type: Full-Time - M-Fr 10a-7p

Salary: $ 95,000 - $105,000

As a Clinical Liaison VIPC, Inpatient Hospice Admissions RN, you will play a key role in identifying patient needs and facilitating seamless transitions into hospice services.

In this role, you'll need the heart of a nurse and a polished business savvy to interact with physicians and healthcare leadership. When hospital patients transfer to hospice, they may stay in the hospital, but care will transfer to the AccentCare Hospice team.

What You Need to Know

Key Reponsibilities:
  • Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily in-person collaboration and communication.
  • Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
  • Conduct patient assessments and coordinate the admission process.
  • Partner with Admissions, Clinical, and Care Teams for smooth transitions.
  • Advocate for patients and families during goals-of-care conversations.
  • Participate in interdisciplinary planning, training, and outreach.
  • Uphold ethics, quality, and continuous learning.
  • Meet expectations for response times, referral coordination, and communication.
