We are currently seeking a Licensed Practical Nurse (LPN/LVN) to join our Skilled Nursing team and make a meaningful difference in residents' daily lives. The Infection Preventionist (IP) is responsible for the facility infection prevention and control program (IPCP) which is designed to provide a safe, sanitary and comfortable environment , and to help prevent development and transmission of communicable diseases and infections.

** Full-Time ** 8:00 AM - 5:00 PM ** $34 - $47/hr. (DOE)
** Schedule: Sunday - Wednesday & Fridays (Schedule negotiable)

In this role, you will:

• Collaborate with Facility Leadership and the Medical Director to develop, implement and evaluate annual infection prevention goals and action
• Foster effective working relationships and consensus building.
• Partner with facility leaders, physicians, and local state and national agencies on activities related to infection prevention.
• Conduct outbreak investigations
• Plan, organize, develop, coordinate, and direct the infection prevention and control program and its activities in accordance with current federal, state, and local standards, guidelines, and regulations that govern such programs.

To thrive in this role, you should have:

• Must meet currently or have completed national training requirements for Infection Prevention and Control
• Current LPN/LVN license in the state of practice
• PointClickCare (PCC) software experience
• Minimum 6 Mos. experience in Skilled Nursing (current or recent)
• Current/prior experience in post-acute, sub-acute, or senior care nursing preferred
• Current CPR certification
