The ICAR unit at Inova Schar Heart and Vascular is looking for an Nurse Practitioner/Physician Assistant (APP) to join their team! Monday - Friday (four 10 hour shifts). No weekends, nights, on-call or holiday requirement.

  • Provide/manage pre and post procedural patient care for EP, Interventional cardiology, structural heart, vascular and IR procedures
  • Assist with ensuring timely discharge/transfer from ICAR, unit flow and utilization
  • Work with anesthesia and proceduralists in pre procedure, PACU and recovery as appropriate to manage patient care
  • Placing post procedure orders and care management orders for inpatients and outpatients
  • Primary support/contact for RNs for patient needs/complications/care pre and post procedures (reducing need for rapid responses and increased coordination and communication for Code Stroke, MSET responses when necessary). Allows physicians to stay on schedule and perform planned procedures - communicates with physicians as needed. Including/not limited to:
    • Chronotropic and vasopressor low dose initiation and management
    • Arterial and venous hemostasis
    • Hemodynamic management
    • Management of arrythmias
    • Pain management
    • Electrolyte management
    • Air way management in PACU
    • Review of CXR/rule out pneumothorax
    • Assist in coordination of emergent care and transfer to appropriate level of care/team
  • Order routine follow-up procedures for IR (nephrotic, biliary or abscess tube exchanges)
    • Order follow-up CT scans if necessary
  • Help with patients requiring admission from ICAR until handed off to the floor staff
    • Patient becomes septic after tube change
    • Patient's RHC shows volume overload requiring aggressive IV diuresis
    • Any complication requiring overnight stay or planned overnight stays if applicable
    • Coordination of care with other teams, orders, level of care, etc. providing smooth and safe transition of care for patients
  • Working on POCUS for bedside echocardiogram, rule out procedural complications as part of evaluation of events, symptoms

Minimum Requirements:
  • Graduation for an accredited NP or PA program. Must be approved to practice as a Nurse Practitioner or Physician Assistant in the state of Virginia. Current prescriptive privileges and DEA license required. BLS and ACLS for Healthcare Provider from AHA required with expiration date greater than 90 days from date of beginning practice.
    • Nurse Practitioner: Master's Degree or Doctorate of Nursing Practice required. License to practice as a Registered Nurse required. Certification in practice from a nationally recognized certifying body required. ACNP, AGACNP certification preferred based on population served.
    • Physician Assistant: Graduate from an accredited Physician Assistant program required; Master's Degree preferred. Certification from the National Commission on Certification of Physician Assistants preferred.
  • Demonstrate appropriate critical thinking and interpersonal skills when working in a fasted paced, high acuity work environment. Able to perform a thorough history and physical assessment.
  • VA License
  • EPIC experience preferred
  • Certification - Basic Life Support prior to start; Advanced Cardiovascular Life Support (for Critical Care) upon start; Pediatric Advanced Life Support (for Pediatrics) upon start
  • Licensure - Licensed Physician Assistant or Nurse Practitioner; If Nurse Practitioner: RN Compact License(depending on State of primary residence) AND a license (or eligibility for) as a nurse practitioner or physician assistant in the Commonwealth of Virginia AND a "Practice Agreement" signed by sponsoring/ supervising physician(s), AND a written "Scope of Practice/Protocol" signed by the supervising physician, AND "Drug Enforcement Administration License" (if prescribing controlled substances).
  • Experience - No prior experience required; APP experience highly preferred.
  • Education - For Nurse Practitioners: Bachelor's Degree in Nursing and Master's Degree in Advanced Clinical Practice or graduate of a post-masters certificate program for nurse practitioners, OR certificate from an education program that is accredited by the Council of Nurse Anesthesia Educational Programs/Schools, American College of Nurse Midwives, American Association of Colleges of Nursing of the National League for Nursing; For Physician Assistants: Graduate of an accredited Physician Assistant program
