POSITION SUMMARY
Assist individuals who are experiencing homelessness or at risk of homelessness who have complex health or behavioral health conditions with applying for and obtaining sustainable income through government programs such as Supplemental Security Income (SSI), or Social Security Disability Insurance (SSDI), or Cash Assistance Program for Immigrants (CAPI). Act as an advocate for HDAP clients throughout the benefits advocacy process.
DUTIES & RESPONSIBILITIES
1. Conduct initial and follow-up interviews of applicants and clients in the office, on the phone and, potentially, at outreach locations outside of the office.
2. Evaluate clients for eligibility for the Housing & Disability Advocacy Program.
3. Work directly with clients to gather all necessary information to complete applications for benefits. Draft thorough and complete applications for disability benefits.
4. Request and evaluate medical evidence.
5. Prepare letters and forms related to client's disability benefits applications.
6. Develop and maintain good working relationships with a variety of healthcare providers and government and social service agencies.
7. Regularly communicate with SSA, DDS, and/or DPSS offices/staff once applications have been submitted, and ensure deadlines are met, and necessary documents are submitted as applicable.
8. Regularly communicate with clients regarding status updates for the cases and in order to ensure that clients are aware of any necessary follow-up in order to ensure the greatest likelihood of approval for disability benefits.
9. Complete appeals-level reconsiderations and hearings requests to SSA as applicable.
10. Prepare, maintain, and update clients' hard case files/e-files related to the case for review and record keeping purposes. Ensure timely completion of client case notes.
11. Performs other related duties as assigned.
EDUCATION/EXPERIENCE
EDUCATION/EXPERIENCE:
1. G8: Bachelor's degree in related field with one year of job specified experience.
2. G9: Bachelor's degree in related field with two years of job specified experience.
3. Employees must be willing to acquire any special certifications or attend any training that is determined to be relevant or become required of the position in the future.
4. Candidates with equivalent combinations of education, training and experience will be considered.
MINIMUM QUALIFICATIONS:
1. Ability to gather complete information from clients in a telephone interview or in person interview.
2. Ability to respond to clients in a calm, empathetic and professional manner while courteously directing the conversation to pertinent issues.
3. Excellent written and oral communication skills.
4. Ability to work independently as well as support team efforts, and to strategize and plan to timely and consistently move applications forward across multiple case timelines.
5. Excellent organizational skills to handle individual caseloads of benefits applications.
6. Experience as an advocate, including intake, interviews and advocating for clients.
7. Experience working with individuals who are homeless or at risk of being homeless (e.g., individuals, families, children, Veterans, etc.) who have complex health and/or behavior health conditions.
8. Strong client customer service skills, including working from a trauma-informed perspective with clients.
9. Demonstrated commitment to serving low-income people and/or people experiencing homelessness.
10. Microsoft Office skills.
11. Two years of Experience
12. Have a good understanding of case management and development of case plans for clients.
13. Knowledge of the laws, rules and regulations including policies, procedures, programs, and goals of public social service agencies.
14. Knowledge and skills in the principles of supervision, training and instructional methods and techniques.
15. Experience and demonstrated knowledge of ICWA and Child Welfare programs and procedures.
16. Ability to exercise sound judgment when organizing, directing, prioritizing, and assigning activities including selecting, training, supervising, evaluating, and disciplining subordinate staff.
17. Demonstrated strong computer skills with three plus years using Microsoft Office or similar software and including a strong working knowledge of automated database systems such as MS Access or similar database.
18. Recognition that in order to serve Yurok People one must know as much as possible about Yurok history, culture and values.
