The RoleAt Abby Care, we are redefining how care is delivered at home, but that journey often starts in the hospital. The Clinical Liaison is the face of Abby Care within major hospitals. This is not a role for someone who wants to sit behind a desk or make corporate-level network calls; this is a highly active, embedded role. You will live within the four walls of your assigned hospitals, navigating the floors, nursing stations and discharge planning departments to become an indispensable resource to clinical staff.
Your primary mission is to educate hospital teams on Abby Care's capabilities, build deep-rooted relationships from the ground up, and ultimately drive a high volume of qualified patient referrals. You will act as the crucial bridge between a patient's hospital discharge and their successful transition to home care. Because this is a true growth role, your success, goals, and compensation are directly tied to the sustained referral volume you generate from your assigned facilities.
Your Responsibilities- Embed & Map the Facility: Maintain a highly visible, daily presence within your assigned hospitals. Identify and map out key decision-makers, including discharge planners, social workers, bedside nurses, and attending physicians.
- Educate & Advocate: Serve as the subject matter expert on Abby Care. Conduct formal presentations and informal floor-level education so staff immediately recognize when a patient is a perfect fit for our programs.
- Drive Referral Volume: Actively prospect for eligible patients and build a predictable pipeline. You will be held accountable to strict volume metrics, and your compensation will scale with the referrals you bring in.
- Streamline Transitions: Remove friction in the admissions process. Work closely with hospital staff and the internal Abby Care intake team to ensure a seamless, urgent handoff from the hospital bed to the home.
- Track & Report: Rigorously utilize our CRM to log facility activities, track relationship progress, manage your pipeline, and analyze growth metrics within your assigned hospitals.
What you'll have:- 4+ years of experience in a hospital or healthcare role, with a track record of building relationships across multidisciplinary teams including discharge planners, social workers and nursing staff.
- You have experience being embedded in the floors, nursing stations and discharge planning rooms where decisions actually get made.
- The experience of being genuinely essential. Clinical teams will come to rely on you as a trusted resource who solves placement problems and makes transitions smoother for patients and staff alike.
- Mastery of clinical environments and patient transitions. You'll deepen expertise in hospital workflows, discharge planning and the complexities of moving patients home.
