Hospice Social Worker (MSW, LICSW) to join our interdisciplinary hospice team serving
South King County.
You'll help patients and families navigate the emotional, social, and practical challenges of serious illness and end-of-life care while ensuring they feel
heard, supported, and respected throughout the journey.Compensation- $38-$45/hour based on experience

collaborative, supportive hospice team that understands the value of the work you do.What You'll DoAs a Hospice Social Worker, you'll be an important part of the interdisciplinary team, helping patients and families navigate end-of-life care with
compassion, dignity, and practical support.- Complete psychosocial assessments and develop individualized plans of care
- Provide counseling and emotional support to patients and families
- Help families navigate end-of-life planning, advance directives, hospice services, and community resources
- Provide behavioral health support and crisis intervention when needed
- Advocate for patient needs, preferences, and rights
- Collaborate closely with nurses, providers, chaplains, aides, and other members of the hospice team
- Connect patients and families with community programs and resources
- Complete accurate, timely, and confidential clinical documentation
What You'll Bring- Master's Degree in Social Work (MSW) from an accredited program
- Current Washington State Social Work license: LICSW, LASW, LSWAIC, or LSWAA
- Strong communication, counseling, and interpersonal skills
- Knowledge of community resources and end-of-life care planning
- Ability to travel throughout South King County
- Hospice or palliative care experience strongly preferred
