Position Summary: The
RN Case Manager (RNCM) is someone whose dedicated clinical expertise results in terminally ill patients receiving exceptional end-of-life care. In this role, the RNCM collaboratively works alongside an interdisciplinary team to provide end-of-life care to those needing hospice services in their home or community setting. The RNCM provides individualized case management, admission facilitation, clinical support, continuous care, after hours call, and/or other skilled nursing expertise with chronic and end-of-life care.
Salary: $83,000-95,000 annually
Job Requirements:- Succinctly and concisely convey patient assessments with clinical data to hospice physician who will then determine hospice eligibility
- Provide direct patient care to terminally ill patients; assess, identify and address their individual physical, psychosocial, safety, and comfort needs
- Anticipate, prevent and treat undesirable symptoms or secondary symptoms
- Develop, maintain, and manage an individualized care plan that captures the evolving needs of the terminally ill patient
- Train, instruct and support the patient, family and caregiver
- Document patient problems, nursing assessments, patient goals, care provided and patient/family outcomes from interventions and care provided in a timely manner
- Consult and collaborate respectfully with the interdisciplinary team and others involved in the patient's care
- Prepare and present all needed information for bi-weekly IDG meetings
Qualifications: - Graduate of accredited school of nursing
- Current state of Minnesota RN licensure
- Three years recent nursing experience in acute, long-term care or home care/hospice/public health
- Patient care planning and patient teaching skills
- Written and verbal communication skills
- Critical thinker
- Current Minnesota driver's license, insurance and daily access to reliable vehicle
- Computer literacy
