Registered Nurse (RN) Case Manager to provide high-quality
Hospice care to patients in the comfort of their homes.
This full-time role is ideal for an RN with experience in
Hospice,
Home Health, care coordination, and patient-centered care, who thrives in an independent, field-based environment while collaborating with an interdisciplinary team.
🔎 What You'll DoPatient Care & Case Management- Perform comprehensive nursing assessments and develop individualized plans of care
- Coordinate and manage hospice services in collaboration with physicians and care teams
- Provide skilled nursing care, treatments, and patient education
- Monitor patient progress and communicate changes in condition to physicians
- Help ensure high-quality, compassionate hospice services are delivered consistently.
Care Coordination & Documentation- Maintain accurate, timely clinical documentation and patient records
- Communicate with healthcare providers and community resources
- Ensure access to necessary medical equipment and services
Leadership & Supervision- Supervise and evaluate Home Health Aides (HHAs), CNAs, and LPNs/LVNs
- Provide guidance to ensure quality care delivery and compliance
Additional Responsibilities- Participate in IDG,quality assurance and performance improvement programs
- Attend training and in-service education
- Participate in on-call rotation as required
• What You'll Need- Graduate of an accredited nursing program
- Active Registered Nurse (RN) license - Oregon
- Minimum 2 years nursing experience (hospice, home health or public health preferred)
- Current CPR certification
- Valid driver's license and reliable, insured vehicle

- Excellent communication and documentation skills
- Ability to work independently with strong time management
- Leadership ability and experience with staff supervision
