Providence Hospice is seeking a
RN Hospice Case Manager, Full Time in St. John, IN and throughout Lake County . We are looking for a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life.
$5,000 retention bonus now being offered. Retention Bonus is paid out in increments when milestone hours are met.

- Conduct initial assessments of new patients.
- Travel to patients wherever they reside: Home, nursing facility etc.
Requirements
- Active Registered Nurse (RN) License in the state of Indiana.
- At least 1 year of RN experience, preferably in Hospice or Home Health.
- Drivers License in good standing.
- Reliable transportation.
