Qualifications

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice
• Strong team collaborator with experience working alongside nurses, social workers, and other healthcare professionals
• Proficient in delivering personalized patient care through 1-1 interactions
• Self-sufficient in managing day-to-day tasks including communication and meetings
• Skilled in time management to balance patient visits and documentation
• Familiarity with electronic medical records systems
• Reliable transportation and valid driver's license required for patient visits across a service area

Responsibilities

• Provide patient care directly in private homes and facilities
• Contribute to interdisciplinary teams by offering expertise in psych-social assessments
• Collaborate with team members for effective interventions
• Work in specific roles such as Case Management, Admissions, or Resource
• Manage documentation and communication effectively to ensure compliance with hospice regulations
• Respond swiftly to urgent patient needs, prioritizing visits and meetings
