Responsibilities

  • Provide comprehensive patient care in nursing facilities and homes

  • Utilize Epic EMR for patient documentation and records management
  • Collaborate with multidisciplinary teams to deliver palliative care services

Full Job Description
Position Details:
• Schedule: Fulltime. Monday - Friday. No weekends or after hours
• Locations: Southern Orange County
• Setting: Nursing facilities, patient homes
• Call: Occasionally cover patient care needs after-hours as needed
• EMR: Epic
• Staff: Medical Assistant, nurses, administrative support

Requirements:
• A Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited institution
• Fellowship training completion in Hospice/Palliative Medicine
• Board Certified in Internal Medicine
• Board Certified in Hospice/Palliative Medicine
• 3-5 years of experience practicing in a home health setting is preferred
• Active California medical license (Or ability to obtain)
