Description

A Day in the Life of a Hospice Liaison
As a Hospice Liaison, you'll spend your day building relationships with hospitals, physician practices, skilled nursing facilities, and healthcare providers throughout our service area. You'll educate professionals about hospice eligibility, benefits, and timely referrals while serving as a trusted resource for end-of-life care. Every conversation you have as a Hospice Liaison helps connect patients to the right hospice services at the right time.

Hospice Liaison Responsibilities
  • Develop and maintain referral relationships with hospitals, facilities, physicians, and community providers.
  • Promote Northern Illinois Hospice and increase awareness of hospice services.
  • Educate healthcare professionals on hospice benefits, eligibility, and referral processes.
  • Coordinate with admissions and clinical teams to support seamless patient transitions into hospice care.
  • Conduct presentations, outreach visits, networking events, and in-service education programs.
  • Identify referral opportunities and implement business development strategies.
  • Represent Northern Illinois Hospice throughout the community.

Requirements
  • Bachelor's degree preferred.
  • Minimum two years of healthcare sales or marketing experience.
  • Hospice or palliative care experience preferred.
  • Nursing or social work background a plus.
  • Strong presentation, communication, and relationship-building skills.
  • Ability to confidently discuss hospice services with healthcare professionals.

Schedule & Compensation

This full-time Hospice Liaison position offers a competitive salary of $79,000-$114,000, based on education and experience.

Expected Hours of Work: Full time, Monday - Friday,

Salary Description

The expected base salary range for this role is $79,000 - $114,000.However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Additionally, this role may be eligible for other forms of compensation and benefits.
