Are you a driven healthcare professional who loves building connections and making a meaningful impact? Northern Illinois Hospice is looking for an exceptional Hospice Liaison to join our award-winning team and help patients and families access the compassionate hospice care they deserve.

A Day in the Life of a Hospice Liaison
As a Hospice Liaison, you'll spend your day building relationships with hospitals, physician practices, skilled nursing facilities, and healthcare providers throughout Boone, DeKalb, and Ogle counties. You'll educate professionals about hospice eligibility, benefits, and timely referrals while serving as a trusted resource for end-of-life care. Every conversation you have as a Hospice Liaison helps connect patients to the right hospice services at the right time.

Hospice Liaison Responsibilities

• Build and maintain strong relationships with healthcare providers, facilities, and community partners while developing new referral partnerships throughout the service area.
• Execute strategic outreach and business development activities to increase awareness of hospice services and connect patients and families with appropriate care.
• Collaborate with the Director of Business Development to achieve referral and growth goals through targeted marketing, relationship-building, and community engagement efforts.
• Maintain a thorough understanding of local healthcare markets, referral sources, and community resources to identify opportunities and respond to evolving patient and partner needs.

Requirements

• Associate's degree preferred or equivalent combination of education and relevant professional experience.
• Two (2) years healthcare sales or marketing experience.
• Hospice or palliative care experience a plus.
• Highly energetic, self-motivated, and relationship-focused, with the initiative, persistence, and enthusiasm to build and maintain strong community partnerships.
• Strong communication skills with the ability to educate and engage healthcare professionals regarding hospice services and appropriate referrals.

Salary Description

We support our employees' wellbeing and regularly benchmark with other companies in our industry to offer an extensive Total Reward package for this position. The expected base salary range for this role is $79,000 - $114,000.However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Additionally, this role may be eligible for other forms of compensation and benefits.
