Halcyon Home is seeking a compassionate and professional Registered Nurse with hospice leadership experience to serve as our Hospice Director of Nursing and support our growing geriatric hospice population in Tomball, TX.

Essential Functions:

As the Hospice RN, Director of Nursing (DON), you will be responsible for overall administration of the clinical departments and monitoring of appropriate staffing and productivity. You will lead a team of skilled nurses and caregivers, ensuring the delivery of exceptional patient care and maintaining the highest standards of professionalism and customer service.
  • Directs and coordinates clinical departments; assumes responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations. (Conditions of Participation).
  • Promotes the philosophy and administrative policies of the Hospice, in an effort to ensure quality of patient care.
  • Participates in activities relevant to professional services including development of qualifications and assignment of the Hospice personnel.
  • Supervises and provides direction to subordinates, in an effort to ensure quality, compliance with the Plan of Care, assessment and reassessment of patients' needs, and continuity of services by appropriate health care personnel.
  • Monitors the QAPI Program and assures appropriate corrective measures are performed.
  • Promotes compliance with all fiscal intermediary and/or other third- party payors, through education, coaching, and other assistance as necessary.
  • Participates as a member of the Interdisciplinary Group/Team (IDG/T), assists in development of the IDG/T plan, and participates in QAPI activities.
  • Monitors systems and identifies problem areas to Administrator both verbally and through written reports.
  • Meets mandatory continuing education requirements of the Hospice and licensing board.
  • Promotes and educates regarding the concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
  • Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.


Requirements

  • Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing (MSN) preferred.
  • Current license as a registered nurse (RN) in Texas and/or in accordance with the Board of Nurse .
  • Minimum of 2 years of nursing experience, with 1 years in a leadership role in hospice or palliative care.
  • Strong understanding of hospice regulations and standards.
  • Home Care Home Base experience strongly preferred.
  • Proven track record of delivering excellent customer service.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Reliable transportation. Valid and current auto liability insurance.
