Hospice Registered Nurse (RN), you will apply critical thinking and clinical expertise to perform comprehensive assessments, develop personalized care plans, and deliver high-quality, compassionate care. Hospice Admission RN, you will also be responsible for initiating the hospice plan of care and ensuring clear, timely communication in accordance with established guidelines.

What You'll Do- Provide skilled nursing care in accordance with physician orders and organizational policies
- Perform comprehensive assessments and develop individualized care plans
- Collaborate with physicians, therapists, social workers, and home health aides
- Supervise and support LPNs and Home Health Aides
- Ensure compliance with regulatory standards and contribute to quality improvement initiatives
- Promote patient satisfaction and positive health outcomes
What You Bring- Current RN license (BSN preferred)
- Minimum 1 year of nursing experience (required in California)
- Home healthcare experience preferred
- Valid driver's license and reliable transportation
- Strong computer skills and a commitment to ethical, mission-driven care
