Job Profile Summary

This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. In addition, this role focuses on performing the following Home Care Nurse duties: Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed. Provides professional nursing care to patients. Requires a nursing license. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. An experienced level role that applies practical knowledge of job area typically obtained through advanced education and work experience. Works independently with general supervision, problems faced are difficult but typically not complex, and may influence others within the job area through explanation of facts, policies and practices.

Job Overview

This position is responsible for patient assessments and admissions to Hospice in the hospital setting while ensuring continuity of care between the admitting process and transition to ongoing care by the hospice interdisciplinary team. This is an external facing position working closely with the Tufts Medical Center and the Boston community. As a key member of the health care team, this position must interact courteously and effectively with patients and their families, as well as with co-workers from all Agency departments, community resources, and with patients' physicians. This position carries out their responsibilities consistent with Agency policies, mission, and vision.

Job Description

Minimum Qualifications:

1. Candidates must hold an active RN license in the state where services are provided and may not work in states where they are not licensed. If additional state licensure is required, it must be obtained within sixty (60) days

2. Valid state issued Driver's License.

Location: MelroseWakefield Hospital

Hours: Full-Time, 40hrs/week.

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

1. Collaboratively works closely with all Tufts care teams, identifies, establishes and maintains positive working relationships with all facility contacts.

2. Collaboratively works with care teams to identify patients potentially hospice appropriate.

3. Evaluates patients for potential hospice care needs and determines if hospice benefit status is appropriate, as requested by hospital clinical staff, or physicians

4. Responsible to guide the patient and family through the admission process.

5. Understands and stays current regarding hospice conditions of participation, reimbursement options and criteria for services.

6. Responsible for hospice in-service education programs as assigned.

7. Responsible for facilitating family meetings, obtaining consent/election form signatures and gathering medical data for review by Hospice physicians.

8. Dependent on volume may complete hospice admission and/or outreach in the greater Boston community, if needed.

9. Seeks opportunities to interface with referring and non-referring health care professionals, patients, families and the community, to promote Merrimack Valley Hospice services.

10. Collaboratively works with agency staff to ensure coordinated hospice discharges or admit GIP at Tufts, if appropriate

11. Arranges for durable medical equipment, and medical supplies needed to meet patient care needs.

12. Completes required paperwork and data entry into electronic medical record by the end of each business day.

Physical Requirements:

1. While performing the duties of this job, the employee is required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell.

2. The employee must regularly lift and/or move 25 lbs and occasionally lift and/or move up to 100 lbs.

3. The employee in this position frequently communicates internally and externally.

4. Must be able to exchange accurate information via phone, email and/or in person.

5. Specific vision abilities apply including close vision, distance vision and ability to adjust focus.

6. Noise level in the work environment is usually moderate.

7. May be exposed to weather conditions prevalent at the time.

1. Ability to effectively communicate with patients, families, physicians, and healthcare team.

2. A team player with decision-making, process management, planning, tracking follow-through and analytical abilities.

3. Knowledge of hospice conditions of participation and eligibility criteria.

4. Excellent interpersonal skills.

5. Ability to function well in very busy situations.

6. Good organization skills.

$87,360.00 - $98,603.60
