Hospice Admission Position Summary and Details
  • Position Summary: As a member of the dedicated admission team, the Admission Nurse has knowledge and competency related to admission compliance requirements and the hospice admission process. Works closely with the attending physician, hospice medical director, patient, and family on the initial assessment, care planning and care implementation for the patient.
  • When a patient and their loved ones are considering hospice care, our Admission Nurses help them throughout the admission process. They educate on the hospice care philosophy and explain the services our company can provide to improve the end-of-life care experience. Our admission nurses complete an average of 1 to 2 admissions a day.
  • Pay: $39.38 Hourly. Full-Time Non-Exempt Position, 40 Hours Weekly/80 Hours Bi-Weekly
  • Schedule: Monday- Friday 8:30 AM- 5:00 PM (working 12:00 PM- 8:30 PM is also an option). There would also be an additional rotational on-call schedule that includes some evenings, weekends, and holidays. As we have an Admission Nurse that works weekends, the weekend on-call would be limited.
  • Location: Will be based out of our Belleville, IL Patient Services Office. Would visit patients and travel within the Belleville office's service territory which include the following Illinois counties: St. Clair, Monroe, Randolph, Washington, Madison, Clinton, Bond, Montgomery, Macoupin, Jersey, and Calhoun.
  • We visit our patients wherever they may be located, whether it be in their homes, long-term care facilities, nursing homes, hospitals, etc. As a result, a valid driver's license, current automobile insurance, and reliable transportation are requirements of the position. The company reimburses .70 per mile for driving personal vehicles.
  • Hospice of Southern Illinois is certified by the State of Illinois and is in compliance with the registration requirements of the Illinois Equal Pay Act.

Education
  1. Graduate of State-approved school of nursing
  2. Associate Degree in Nursing or Diploma in Nursing required. Bachelor of Science in Nursing, preferred.

Experience

  1. One (1) year experience in hospice preferred.

Other Qualifications
  1. Current Illinois RN license.
  2. Certification in hospice and palliative care preferred.
  3. CPR certification preferred.
  4. Excellent clinical assessment, organizational and interpersonal relationship skills.
  5. A working knowledge of Medicare/Medicaid, hospice regulations, and third-party payor expectations is preferred.
  6. Reliable means of transportation.
  • Physical Requirements: Performs repetitive tasks. Physical tasks including bathing, transferring, lifting, and turning of patients in the home or home-like setting. Prolonged periods of sitting at times. Exerts up to 100 lbs. of force occasionally, and/or up to 50 lbs. frequently and/or 20 lbs. constantly. Requires physical flexibility of bending at knee and squat-bend.
    The Admission Nurse manages stress appropriately, makes decisions under pressure, handles multiple priorities, works with an Interdisciplinary Team and manages anger/fear/hostility/violence of other appropriately. This position requires clarity of vision >20 inches and
  • Working Conditions: The Admission Nurse spends approximately 50% of his/her time in an air-conditioned environment with varying exposure to excessive humidity and noise. The remaining 50% of his/her time is spent in an automobile or patient home, which may or may not have heat/air conditioning. The position has potential for exposure to malodorous odors, infectious body fluids from patients, animal pet hair, allergens in the home environment, some exposure to noxious smells from cleansing agents, and exposure to communicable diseases and traffic.
