Base Salary: $95,000-$110,000/year
Sign-On Bonus:$3,000

Metro is looking for a motivated and passionate full-time Physical Therapist to provide in-home care throughout Eastern Suffolk County.

Responsibilities
  • Accept and treat patients within your designated geographic area
  • Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans
  • Develop a plan of care and treatment goals in consultation with the physician, patient, and family
  • Plan, prepare, and implement customized therapy programs to restore function and promote independence
  • Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
  • Provide timely and accurate documentation for each visit
  • Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
  • Deliver care in compliance with agency policies and professional practice standards


Qualifications
  • Graduate of an accredited Physical Therapy program
  • Licensed Physical Therapist in the state of New York or eligible for licensure
  • Home health experience is preferred but not required - we will provide you with a mentor to help with documentation and compliance
  • Ability to travel to patients' homes within the assigned geographic territory


Additional Information

The anticipated base salary range for this position is $95,000-$110,000 annually. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.

This position is eligible for a $3,000 sign-on bonus. Additional terms and conditions may apply.
