Job Summary

The Home Health Wound Nurse (RN) is responsible for providing specialized wound care services to patients in their homes. This role focuses on assessing, planning, implementing, and evaluating individualized wound care treatment plans in collaboration with physicians and the interdisciplinary healthcare team.

The Wound Nurse plays a critical role in promoting wound healing, preventing complications, identifying changes in patient condition, and educating patients and caregivers on proper wound care management.
Qualifications

Required Qualifications
  • Current and unrestricted Registered Nurse (RN) license in the State of Connecticut
  • Current BLS (Basic Life Support) certification
  • Minimum of one (1) year of nursing experience in home health, wound care, or a related clinical setting
  • Strong clinical knowledge of wound assessment, treatment, and infection prevention
  • Strong communication, organizational, and clinical documentation skills

Preferred Experience
  • Experience managing chronic wounds, pressure injuries, diabetic foot ulcers, surgical wounds, and other complex wound conditions
  • Experience with negative pressure wound therapy (wound VAC)
  • Previous experience working in a home healthcare setting
  • Experience providing patient and caregiver education related to wound management
  • Strong electronic health record (EHR) documentation experience
What You'll Do

As a Home Health Wound Nurse, you will:
  • Conduct comprehensive wound assessments, including evaluation of wound type, location, stage, size, severity, drainage, and overall condition
  • Develop and implement individualized wound care plans in accordance with physician orders, clinical assessments, and established standards of care
  • Perform skilled wound care treatments, including dressing changes, topical treatments, wound VAC management, and other prescribed interventions
  • Monitor wound healing and patient progress, identify changes or complications, and communicate concerns to the appropriate healthcare provider
  • Assess patients for signs and symptoms of infection and respond appropriately in accordance with physician orders and agency protocols
  • Educate patients and caregivers on wound care techniques, infection prevention, skin integrity, and proper home care practices
  • Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary healthcare team to ensure coordinated and comprehensive patient care
  • Document wound assessments, treatments, patient responses, education, and outcomes accurately and timely in the electronic health record (EHR)
  • Participate in interdisciplinary care discussions and provide clinical updates and recommendations regarding patient progress
  • Promote patient safety, independence, and adherence to the established plan of care
Compensation

Salary: $100,000 - $110,000 per year
