Home Health RN Responsibilities:
- Conductinitial and ongoing patient assessments in the home setting
- Developand update individualized care plans in collaboration with the care team
- Administermedications, treatments, and injections as prescribed
- Performwound care and dressing changes and other complex care
- Monitorvital signs and symptoms, reporting changes to the physician
- Educatepatients and caregivers on disease processes, medications, and self-care
- Coordinatecare with physicians, therapists, and other healthcare professionals
- Documentall visits and care provided in the EMR
- Ensurecompliance with agency policies, Medicare/Medicaid, and other regulations
- Participatein case conferences and interdisciplinary team meetings
- Provideemotional support and promote patient independence
Requirements:
- Washington RN licensure
- Home Health/Hospice agency experience preferred, but not required
- Current BLS certification
- Valid driver9s license and insured vehicle
