Home Health Registered Nurse (RN)Case Manager

Newburyport Territory/Full Time (Monday through Friday 8a-5p)

As we continue to grow in the Newburyport market, we are expanding our clinical team and seeking a dedicated Registered Nurse (RN) 6 Home Health who is ready to be part of a mission-driven organization that values clinical excellence, autonomy, and meaningful patient relationships.

What Youll Do

As a Home Health Registered Nurse, you will manage a caseload of patients and provide comprehensive nursing care throughout their healthcare journey.

Responsibilities include:

  • Completing skilled nursing assessments and developing individualized plans of care

  • Performing admissions, routine visits, recertifications, and discharge visits

  • Evaluating patients physical, psychosocial, environmental, and educational needs

  • Coordinating care with physicians, caregivers, and interdisciplinary team members

  • Monitoring patient progress and identifying changes in condition

  • Updating and revising care plans to ensure optimal outcomes

  • Educating patients and families on disease management, medications, and self-care techniques

  • Maintaining accurate and timely clinical documentation

  • Helping prevent avoidable hospitalizations through proactive intervention and patient education

  • Delivering compassionate, patient-centered care while promoting safety and independence

Requirements:
  • Current, unrestricted Massachusetts Registered Nurse (RN) license

  • Minimum of one (1) year of nursing experience in a clinical care setting

  • Ability to commit to at least one full day (8 hours) per week

  • Valid drivers license, reliable transportation, and current auto insurance

  • Willingness to travel throughout the assigned territory

  • Physical Requirements - Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
