Key Responsibilities
- Perform comprehensive initial and ongoing patient assessments in the home setting.
- Develop individualized care plans based on patient needs, physician orders, and regulatory requirements.
- Deliver skilled nursing interventions including wound care, medication management, IV therapy, catheter care, and disease-specific education.
- Monitor patient progress, identify changes in condition, and communicate updates promptly to physicians and interdisciplinary team members.
- Coordinate services with nurses and caregivers to ensure continuity of care.
- Complete timely, accurate documentation including assessments and visit notes.
- Educate patients and caregivers on disease processes, medications, safety, and self-management strategies.
- Participate in case conferences, quality initiatives, and on-call rotation as assigned.
Qualifications
- Current and unrestricted Registered Nurse (RN) license in the state of practice.
- Five (5) years of clinical experience (Preferred)
- Two (2) years of experience in a home health agency (Preferred)
- Strong assessment, critical-thinking, and communication skills.
- Ability to work independently, manage a field-based schedule, and prioritize multiple tasks.
- Knowledge of Medicare home health regulations and documentation standards preferred.
- Valid driver's license and reliable transportation.
Preferred Skills
- Familiarity with home health EMR systems.
- Background in wound care, chronic disease management, or geriatrics.

Job Type: Full-time
Pay Range:
- Annual salary range of $80,000 and $85,000
