Join out BrightStar Care Clinical Team as a Home Health RN Case Manager!

Key Responsibilities
  • Perform comprehensive initial and ongoing patient assessments in the home setting.
  • Develop individualized care plans based on patient needs, physician orders, and regulatory requirements.
  • Deliver skilled nursing interventions including wound care, medication management, IV therapy, catheter care, and disease-specific education.
  • Monitor patient progress, identify changes in condition, and communicate updates promptly to physicians and interdisciplinary team members.
  • Coordinate services with nurses and caregivers to ensure continuity of care.
  • Complete timely, accurate documentation including assessments and visit notes.
  • Educate patients and caregivers on disease processes, medications, safety, and self-management strategies.
  • Participate in case conferences, quality initiatives, and on-call rotation as assigned.

Qualifications

  • Valid driver's license and reliable transportation.

Pay Range:
  • Annual salary range of $80,000 and $85,000
