What you'll do- Provide skilled nursing care in the home following discharge from a hospital, rehab, or nursing facility
- Conduct assessments, document progress, and communicate changes to the physician
- Oversee LPNs, CNAs, and aides supporting your patients
- Educate patients and families on care plans, medications, and home safety
- Coordinate care goals and manage discharge from services
What we're looking for- Active RN license in Oklahoma
- At least one year of nursing experience; home health experience preferred
- CPR certification
- Strong clinical judgment, communication skills, and a genuine passion for patient-centered care
- Reliable transportation
