Qualifications

• Graduate of an accredited nursing school with 1-2 years of recent acute care experience in a clinical setting.
• Current state licensure and CPR certification required.
• Preferred: Bachelor's degree and 1 year of home health care experience.
• Strong observation and communication skills, with problem-solving abilities and basic math knowledge.
• Licensed driver with an insured vehicle in good working condition.

Responsibilities

• Assess new patients' conditions, histories, and home care needs.
• Collaborate with patients and families to build personalized care plans.
• Revise care plans as patients' statuses evolve.
• Administer prescribed medications and treatments effectively.
• Educate patients and families on home care management.
• Coordinate communication with physicians and update care every 60 days.
• Engage with community health partners involved in patient care.
• Plan and execute discharge processes when patients are ready.
• Arrange necessary equipment or services for patient care.
• Conduct bi-weekly check-ins on home health aide services.
