🩺 A Day in the Role

As a Home Health Registered Nurse, you will:
  • Complete initial and ongoing nursing assessments in patients' homes
  • Manage an assigned caseload and coordinate each patient's care
  • Develop, implement, and update individualized Plans of Care
  • Provide skilled nursing treatments in accordance with physician orders
  • Monitor patients' conditions, medications, symptoms, and responses to treatment
  • Identify and promptly communicate changes in condition or safety concerns
  • Educate patients and caregivers about medications, treatments, diagnoses, and self-care
  • Coordinate services with physicians, therapists, social workers, aides, and other care partners
  • Help patients and families access appropriate clinical and community resources
  • Take proactive steps to reduce avoidable hospitalizations and emergency department visits
  • Maintain accurate, timely, and complete clinical documentation
  • Follow all clinical, safety, regulatory, and HIPAA requirements


You're a Great Fit If You Have
  • A degree or diploma from an accredited nursing program
  • A current, unrestricted Registered Nurse license in the applicable state
  • At least one year of nursing experience in a clinical care setting
  • Strong assessment, communication, organization, and care-coordination skills
  • The confidence to work independently while remaining connected to a clinical team
  • A dependable vehicle, valid driver's license, and current automobile insurance
  • The ability and willingness to travel throughout the assigned territory
  • Physical Requirements: Sit, stand, bend, lift, and move intermittently and be able to lift 50-100 lbs.
