Are you looking for a change from clinical 9-5? Signature Healthcare at Home has an exciting opportunity to add a Physical Therapist to our team! This position covers Skagit County (Mount Vernon, Burlington, Bow, Anacortes, Sedro-Woolley, Hamiliton, Concrete) as the main coverage area. The ideal candidate is someone with home health experience and enjoys work with a high-level of autonomy.
Duties and Responsibilities- Assumes the responsibility for the on-going interdisciplinary and individualized plan of care in partnership with the patient, family/representative and/or caregiver(s).
- Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
- Provides patient, family, and/or caregiver counseling, education, and progress summary based on the attainment of goals.
- Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
- Supervises physical therapy assistant(s) according to organizational policy and in alignment with state regulations.
- Completes daily documentation, scheduling
Knowledge, Skills and Experience- Possess and maintains licensure to practice Physical Therapy in the state of Washington.
- Graduated from an accredited CAPTE or successor organization with at least a baccalaureate degree for legacy programs or a doctorate's degree in physical therapy.
- Possesses and maintains a valid and active BLS CPR certification from the American Heart Association or American Red Cross.
- Possesses and maintains a valid drivers' license and personal automobile with insurance for daily travel as a requirement.
Schedule, Pay and Benefits- Full-time Monday through Friday, flexible scheduling.
- Hourly $50 - 60 /hour DOE + Mileage Reimbursement.
- Medical/Dental/Vision available the 1st of the month after hire.
- Paid company holidays, 2+ weeks PTO and 401k match after 90 days.
