Home Health Manager (RN)

Location: Carson, California, United States
Salary: $125,000-$144,197

Position Summary

The Home Health Manager is responsible for leading and overseeing skilled home healthcare services for older adults living at home, as well as coordinating care provided in nursing homes and board-and-care facilities. This leadership role manages home health staff, ensures high-quality clinical care, supports interdisciplinary collaboration, and maintains compliance with organizational policies and healthcare regulations.
Key Responsibilities
  • Lead and manage the home health department, ensuring efficient workflows and adherence to established policies and procedures.
  • Recruit, train, mentor, and supervise home health team members.
  • Evaluate the effectiveness of nursing assessments and interdisciplinary team (IDT) participation.
  • Ensure home care nurses deliver high-quality care and accurately document nursing interventions in accordance with individualized care plans.
  • Oversee the quality of care provided in participants' homes, nursing homes, and board-and-care facilities.
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate comprehensive participant care.
  • Monitor quality metrics, regulatory compliance, and continuous performance improvement initiatives.
Qualifications
