HOME HEALTH & HOSPICE DIRECTOR

JOB DESCRIPTION

Job Summary: To plan, organize, direct, control, and evaluate the home health and hospice services and operations so the quality of care is optimized to reflect the philosophy of Bethesda. Responsible for the patient management, scheduling, leadership responsibilities such as overseeing intake referrals, budget management, quality management and overall operations of the home health and hospice services which encompasses staff of RNs, LPNs, home health and hospice aides, social workers, and scheduling/office support staff.

Job Relationships:
  1. Reports to: Vice President of Operations
  2. Supervisory Responsibilities: Nursing staff, RNs, LPNs, Home Health/Hospice Aides, Scheduling Coordinator/Office Assistant, Social Worker
  3. Internal Relationships: Accounts Payable, Billing, Bethesda Social Service, Adult Day Director, Housing Manager and Coordinators, Human Resources, Therapy
  4. External Contacts: Medical Director, Physicians, Pharmacists, MDH personnel, discharge planners/referral sources, clients/patients and families, general community.

Hours of Work: 40 hours per week, 8:00 am to 4:30 pm.; Monday through Friday. Must be available via telephone during business hours and as needed after normal business hours.

Dress Code: Professional attire appropriate to a clinical and administrative environment; identification badge required at all times.

Essential Requirements of Work:
  1. Education:
    1. Must be registered and licensed with the State of Minnesota as a RN and must maintain a current and unrestricted licensure.

Principal Job Responsibilities:

Responsibility: Select and assign personnel.

Tasks:
  1. Interview, selection and hiring of all staff including nurses, home health and hospice aides, homemakers, and ancillary staff.
  2. Ensure all staff are appropriately licensed and/or certified in the State of Minnesota.
  3. Review job description, wage, and hours of work with each new employee at the time of hire.
  4. Coordinate with human resources to ensure background checks are completed.
  5. Ensure that all staff remain current with necessary continuing education.
  6. Orientation and training of all new staff.

Responsibility: Monitor staff work performance.

Tasks:
  1. Provide opportunities for employees to attend in-services and workshops in compliance with State and Federal regulations.
  2. Hold staff meetings to review policies and procedures or changes. Document content and attendance.
  3. Investigate and document all employee incidents.
  4. Monitor all worker compensation cases to promote a timely return to full work duties.
  5. Assign or complete all performance evaluations.
  6. Provide corrective action as necessary to ensure consistency and quality care.
  7. Ensure that all supervision visits of staff are completed as mandated by state and federal regulations.
  8. Complete random quality checks with clients to ensure client satisfaction with staff performance.

Responsibility: Assure quality client care.

Tasks:
  1. Oversee the referral, admission and scheduling process.
  2. Protect and promote the rights of each client, including keeping clients free from abuse and neglect. Investigate, evaluate, report and follow up on any complaints received.
  3. Develop, implement, review, and revise policies and procedure that guide and support the provision of services.
  4. Assure compliance with State and Federal regulatory requirements.
    1. Prepare and update survey readiness book.
  5. Develop, implement, review and revise a Quality Assurance/Improvement Program
  6. Oversee the RN Coordinators to assure all required assessments and documentation is complete and accurate.

Responsibility: Financial management of Agency Tasks:
  1. Complete payroll for each employee each pay period and submit to payroll office.
  2. Review financial and billing reports at the end of each month. Monitor revenue and expenses.
  3. Coordinate with billing office to ensure that all necessary paperwork is completed in order to complete monthly billing of services.

  6. Maintain all contracts with outside agencies for billing of client services.
  7. Maintain all required business licenses.
  8. Work with Vice President of Operations to develop annual budget.

Responsibility: Market and promote Home Health and Hospice services.

Tasks:
  1. Maintain ongoing liaison with staff, clients, family members, and community.
  2. Work with hospital discharge planners, nursing home social workers, county social workers, and physicians to determine appropriate referrals.
  3. Assure accuracy of public information materials and activities. Promote services at health fairs and other community events as appropriate.
  4. Coordinate with community director to plan and schedule community events to promote home care services and generate referrals.
