Performs comprehensive assessments, evaluations and re-evaluations on patients to establish plan of care and goals. Performs patient treatment according to Physical Therapy procedures of modalities, therapeutic exercise, gait training and other accepted methods. Communicates pertinent information verbally or written to physician, other staff members, patients/caregivers. Supervises and directs Home Health Aide’s care in relationship to the patient’s rehab program. Demonstrates active participation in home care department functions.

EDUCATION, EXPERIENCE, TRAINING

1.    Bachelor, Masters, or Doctorate Degree in Physical Therapy from an accredited program.

2.    One year recent clinical experience.   Home Care experience preferred.

3.    Current license as a Physical Therapist. 

4.    Current BLS certification. 

5.    Current driver’s license, car insurance, car registration, and reliable automobile.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $35.71 to $51.72. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
