Job Summary: The Home Care Nurse Manager ensures all the home care in-home aide requirements for nursing are met for the Piedmont Health Home Care agency, and that high quality home care services are provided to Piedmont Health SeniorCare participants. Specially, this position does home care admissions and assessments, develops home care plans of care, conducts supervisory visits according to the regulations, and ensures quality services and documentation of home care services. Moreover, the Home Care Nurse Manager develops and provides education to in-home aides. This position works collaboratively with the Home Care Manager to ensure compliance, high quality, and efficient and effective operations. The home care nurse manager collaborates with the PACE team on coverage of home care services, quality, and effective communication between the two entities and with PACE participants and families. The position maintains policies and procedures for home care and this position serves on-call rotation for home care.
Minimum Qualifications -Education/Experience: Graduate of an accredited school or nursing. 1 year experience working with the fail or elderly population required. Preferred: Has practiced nursing in the last 3 years; 3 years RN experience with at least 12 months experience working with the frail or elderly population. Prefer experience in home care setting.
- Must possess knowledge and skills necessary to employ the nursing process to perform care for patients
- Coordinating home care services
- Assessment of participants within their home environment
- Serves as nursing liaison to family and participants
- Serves as a liaison to contracted home care agencies.
- Oversees utilization of home care services at PACE.
- Meet a standardized set of competencies for the specific position description established by PHSC and approved by CMS before working independently
Type of Employees Supervised: Supervises nurse aides who work for home care agency and administrative scheduling staff. Responsible for audits of home care agencies supervision of their employees.
Licensure, Registry or Certification Required: A RN currently licensed in NC; presents credentials which demonstrate ability to meet performance criteria for the position. Must have a valid driver license and be able to drive to and from the home of participants.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
Salary Range: $79,424 - $102,707
