Job Description:

  • Coordinates systems related to heart failure care delivery, ensuring efficient and effective patient care.
  • Assists in providing staff education related to heart failure to ensure up-to-date knowledge and practices.
  • Evaluates the delivery of care system for heart failure patients to ensure compliance with quality measures.
  • Monitors quality of care issues through team activities and identifies deficiencies in care delivery.
  • Facilitates the Heart Failure team and collaborates with the Cardiology Clinical Team to lead improvement activities.
  • Acts as a clinical liaison between physicians, department directors, and staff regarding heart failure care delivery.
  • Implements corrective actions through education of staff to address identified deficiencies.
  • Assures compliance with quality measures through concurrent and retrospective review.
  • Serves as co-chair of the heart failure team meeting, drafting meeting agendas and minutes.
  • Coaches and mentors staff, providing guidance and support to improve care delivery.
  • Communicates information to team members and acts as a liaison to departments.
  • Participates in heart failure-related activities and provides reports on heart failure and patient care.
  • Performs other duties as assigned.

Knowledge, Skills, and Abilities:
  • Must have the knowledge and skills necessary to provide care that addresses the physical, psychosocial, educational, safety, and age appropriate needs of the patient population served within the department. [Required]
  • Ability to analyze, interpret and present data. [Required]
  • Computer and powerpoint skills [Preferred]

Education:
  • Associate's of Nursing [Required]
  • Bachelor's of Nursing [Preferred]
  • Masters of Nursing [Preferred]

Field of Study:
  • Maintain 8 hours in heart failure specific education annually
