Harm Reduction Outreach Lead Reference No. TWT-SSC-HROL-0826 PURPOSE: Reporting to the Slhexun sun'ts'a' Clinic Manager, the Harm Reduction Outreach Lead is responsible for leading the Ts'ets'uwut Tth'ele' Mustimuhw (Helping Hearts Outreach Team) Harm Reduction health and outreach services and program administration. The Harm Reduction Outreach Lead supports Integration of Harm Reduction and Outreach Services into Primary Care by working as part of the Primary Care Team at Slhexun sun'ts'a' Clinic which is an interdisciplinary team consisting of nurses, support workers, and peers. This is a full-time permanent position that is required to work in the office and in community with moderate local travel required. COMPENSATION AND BENEFITS: Cowichan Tribes offers excellent health and dental benefits and pension plan, with benefits eligibility beginning after one month of employment and pension plan eligibility after three months. The position includes paid training, paid vacation and paid sick leave. Depending on experience, the position starts at $49.43 - $52.41 per hour ($89,966.06 - $95,378.42 annually). RESPONSIBILITIES: Lead Harm Reduction and Outreach Activities: • Develop and implement Harm Reduction and Outreach programs, services, and schedules, including outreach, and community engagement to support community wellness for community members • Collaborate with internal and external partners, including Ts'ewulhtun Health Centre programs such as Kwun'atsustul Counselling and Sa'qw'thut Culturally-Grounded Day Program and Hwialusmutul community health team • Ensure all documentation is completed thoroughly, data entered, and filed appropriately • Stay apprised of most recent Harm Reduction and Outreach health information and best practices and supports ongoing staff training and development • Develop and implement new program activities in collaboration with the Clinic Manager • Advertise program services at a variety of events and meetings • Participate in a variety of internal and external committees and working groups as appropriate Program Administration • Provide professional and ethical guidance, support and leadership to the Ts'ets'uwut Tth'ele' team • Ensure compliance with all internal and external policies and procedures • Consult with Clinic Manager regarding employee complaints, performance and professional development • Advise Clinic Manager on emerging trends, concerns and feedback • Debrief with team regarding program activities • Work in collaboration with Clinic Manager to coordinate program budget, keep accurate records of finances and participate in regular budget reviews and planning. • Participates in development of program funding proposals, ensure program funds are expended appropriately and provide subsequent reporting as needed • Participate in establishing and maintaining program policy, procedures and standards • Supports data collection, tracking and reporting Community Engagement • Ensure the outreach team maintains a strong working relationship with local frontline providers as appropriate which may include Island Health (such as Public Health, YSTAR, ACT, SUIT, etc.), OPS, low- barrier housing sites and shelters, CMHA, CWAV, as well as MCFD, Stsi'elh Stuhw Tu Smun'eem, RCMP, bylaw, Cowichan Tribes security and foot patrol, and other community partners • Stay apprised of local trends, overdose alerts, available programs and services, and frontline concerns in community • Attend frontline provider and care coordination meetings with both internal and external programs regarding clients and program activities when needed • Advocate internally and externally for culturally safe interactions between First Nations clients and the local community, including health services • Participate in a variety of committees and working groups related to supporting community members to access services such as frontline provider care coordination meetings, or other Community Partner meetings as appropriate • Coordinate and assist with off-site wellness events as needed • Attend community events and programs for community engagement • Consult with internal/external partners and other community resources as appropriate • Other related duties as required EDUCATION AND EXPERIENCE: (please attach all required documents) • Bachelor's Degree in a regulated health profession (health administration, nursing, counselling, social work or related) is required • 7 years of progressive clinical or health administration experience or related • 2 years of supervisory experience • An equivalent combination of relevant education and experience in a community health-related setting may be considered • Certification in Basic First Aid / CPR-C / Food Safe Level 1 • Satisfactory Criminal Record Check with vulnerable sector is required • WHMIS certification is required • Valid British Columbia Drivers' License, Drivers' Abstract is required • Experience working in a First Nations community is essential, and a willingness to learn about the language, culture, customs and traditions of First Nations' peoples including Quw'utsun Mustimuhw • Naloxone Train the Trainer certification is an asset KNOWLEDGE, SKILLS, AND ABILITIES: • Knowledge of substance use, addictions, and harm reduction • Knowledge of behavioral and mental health issues associated with substance use, and homelessness • Ability to adapt to changing work environments, work priorities and the needs of a team • Experience working in community and frontline with vulnerable populations is essential • Well-developed written, oral, interpersonal and organizational skills • Demonstrated skills in developing and maintaining positive relationships in the workplace • Demonstrated abilities leading effective teams • Knowledge of Cowichan culture, customs, and traditions • Knowledge of internal and external community resources • Knowledge of prevention, intervention, and treatment techniques and resources • Time management with the ability to multitask different tasks simultaneously
