A concierge-style geriatric practice in the Palm Beach and Boca Raton area is seeking a Nurse Practitioner to provide high-quality, individualized care for a small, dedicated patient panel. This role offers strong clinical autonomy, a balanced mix of home visits and facilities, and the chance to help shape a growing practice.
Key Responsibilities:- Conduct comprehensive in-home geriatric assessments
- Provide ongoing care and follow-up visits through home appointments and facilities
- Collaborate with the physician team as needed while maintaining a high level of independence
- Support care planning, medication management, and chronic disease oversight
- Maintain accurate documentation and utilize EMR tools
