Qualifications:- Graduate of an accredited dental school with a D.D.S. or D.M.D. degree
- Active license to practice dentistry in the State of Wisconsin (or ability to obtain)
- Possess current DEA registration and prescribing privileges
- Basic Life Support (BLS) certification required
