Overview

UH Cleveland Medical Center (UH CMC) is seeking a highly motivated General Cardiology and Heart Failure ICU Clinical Pharmacy Specialist to join our pharmacy team. This position will provide dedicated clinical pharmacy services to the inpatient cardiology consult service and Heart Failure Intensive Care Unit (HF ICU), serving as an integral member of the multidisciplinary cardiovascular care team.

The Clinical Pharmacy Specialist will serve as a subject matter expert in cardiovascular pharmacotherapy, working collaboratively with cardiologists, heart failure specialists, advanced practice providers, nurses, and pharmacy colleagues to optimize medication therapy, improve patient outcomes, and advance quality initiatives within the cardiology and heart failure service lines at UH Cleveland Medical Center.

Position Summary

This General Cardiology and Heart Failure ICU Clinical Pharmacy Specialist will:

• Serve as the pharmacotherapy expert for patients cared for by the inpatient cardiology consult service and Heart Failure ICU.
• Participate in daily multidisciplinary rounds with the cardiology and heart failure teams and contribute to individualized patient care plans.
• Optimize medication therapy through evidence-based recommendations related to cardiovascular and heart failure pharmacotherapy.
• Provide therapeutic drug monitoring and medication management for high-risk and complex cardiovascular therapies.
• Support guideline-directed medical therapy (GDMT) implementation and optimization across the continuum of cardiovascular care.
• Develop and maintain clinical guidelines, protocols, and best practices related to cardiology and heart failure pharmacotherapy.
• Lead and participate in quality improvement, medication safety, and resource stewardship initiatives.
• Support clinical teaching, research, and scholarly activities within the Department of Pharmacy.

Key Responsibilities

• Participate in daily multidisciplinary patient care rounds and clinical discussions with the inpatient cardiology consult service and Heart Failure ICU teams.
• Provide evidence-based recommendations regarding medication selection, dosing, monitoring, and optimization for cardiovascular disease states.
• Assist in the management of complex cardiovascular therapies including anticoagulants, antiplatelet agents, antiarrhythmics, vasoactive agents, inotropes, and advanced heart failure therapies.
• Promote guideline-directed medication therapy for patients with heart failure and other cardiovascular conditions.
• Perform therapeutic drug monitoring and pharmacokinetic consultation when applicable.
• Monitor for and prevent adverse drug events, medication errors, and drug-related complications.
• Develop, implement, and maintain clinical policies, guidelines, protocols, and order sets related to cardiovascular pharmacotherapy.
• Promote adherence to formulary standards and provide guidance regarding appropriate medication utilization.
• Collaborate with physicians, advanced practice providers, nursing staff, and pharmacists to optimize patient outcomes.
• Document clinical interventions and associated outcomes.
• Serve as a resource and educator for pharmacists, physicians, advanced practice providers, nurses, pharmacy residents, and pharmacy students.
• Participate in departmental committees, quality improvement initiatives, research activities, and medication-use evaluations.
• Provide Clinical Pharmacy Services (drug information, drug therapy monitoring, patient specific consultations, healthcare staff education, pharmacoepidemiology and pharmacoeconomic evaluations in selected patient populations)
• Investigate/research/report adverse drug reactions
• Participate in Medication Safety and Therapeutics (MS&T) Committee process: evaluate requests for new drug usage and old formulary drug relevance and review therapeutic classes for relevance, assist in maintaining and updating the system formulary
• Serve as an informational and patient problem solving resource for Clinical Pharmacists and other healthcare professionals
• Provide continuing education to Clinical Pharmacist staff
• Documentation of clinical activities
• Serve as a preceptor for pharmacy residents and pharmacy students
• Actively participate in local and system committees and workgroups pertaining to the Clinical Pharmacy Specialist's area of expertise

Qualifications:
Education

• Doctor of Pharmacy (PharmD) required.
• Completion of a PGY1 Pharmacy Residency preferred.
• Completion of a PGY2 Cardiology, Critical Care, or equivalent experience strongly preferred.

Work Experience

• Advanced training or experience in cardiovascular pharmacotherapy preferred.
• Experience caring for patients with cardiovascular disease, acute decompensated heart failure, or critically ill cardiac patients preferred.
• Experience with multidisciplinary cardiology and/or critical care practice models preferred.
• Experience with quality improvement initiatives and guideline implementation preferred.

• less than 1 year Experience as a pharmacist with training in drug therapy of diseases including the knowledge and application of clinical pharmacology, pharmacokinetics, drug-drug interactions, and pharmaceutics to patients, training in cost analysis/cost evaluation techniques (Preferred)

Licensure

• Licensed or eligible for licensure as a Registered Pharmacist (R.Ph.) in the State of Ohio required.
