Department/Unit/Program Description RMH is a medium sized hospital that provides 24-hour care to Emergency Room patients. Position Summary The Geriatric Emergency Management (GEM) Registered Nurse is an advanced practice nurse who provides expert clinical assessment, intervention, and care coordination for frail older adults at risk of adverse outcomes.
The GEM RN focuses on seniors at risk of functional decline, loss of independence, hospital admission, or long-term care placement. Through comprehensive geriatric assessment and targeted interventions, the GEM RN promotes optimal patient outcomes and supports care in the most appropriate setting, making recommendations for care of frail seniors and support implementation within the ED and across hospital services.
The GEM RN functions as a clinical expert, educator, and system navigator, enhancing linkages between hospital, community, and long-term care services while building geriatric capacity within the ED, the organization, and the broader health system.
A key focus of this role is to support efforts to reduce hospital length of stay through promoting safe discharge planning and alternatives to hospital admission. This includes liaising with internal and external partners-such as hospital staff, primary care providers, Ontario Health at Home Care Coordinators, the Alzheimer Society, Community Paramedics, Community Care Home First, Hospice, and the Mental Health Crisis Team-to coordinate and integrate care plans that facilitate successful transitions, guided by a "home first" philosophy.
This nurse will be based in the Emergency Department, an environment with a high volume of complex patients, and report to the Manager of the Emergency Department.
The Emergency Department is currently seeking a Temporary Full-time Geriatric Emergency Management (GEM) RN to join their healthcare team.

Ending on or before: April 05, 2027
Essential Qualifications• Registered Nurse in good standing with the College of Nurses of Ontario
• Bachelor of Science in Nursing (BScN) required
• Minimum of 3 years' experience in Geriatrics, Palliative and/or Emergency Nursing
• Experience with comprehensive geriatric assessment, discharge planning, and community resource navigation
• Advanced post-baccalaureate education in geriatric care is required, such as:
- Gerontological Nursing Certificate (CNA)
- P.I.E.C.E.S., GPA, delirium/frailty training, or equivalent advanced geriatric education
• Experience leading projects and training including building community linkages
• Proven ability to liaise effectively with interdisciplinary teams and community partners to foster collaboration and achieve complex outcomes
• Proven ability to practice independently and interdependently
• Excellent organizational, interpersonal, time management, problem solving and decision-making skills
• Proven ability to manage a diverse case load
Preferred Qualifications• Member of the Registered Nurses' Association of Ontario (RNAO)
• Post-baccalaureate certificate in nursing (including Masters Degree, certification in Gerontological Nursing or Palliative Care Certificate).
• Continuing education in managing responsive behaviors (i.e., P.I.E.C.E.S certificate, Montessori Dementia Programming)
• Familiarity with program development, evaluation and research methodology
Hours of Work Must be able to work 12-hr shifts, Monday to Sunday, including holidays. Shift hours may change according to departmental requirements and as per the ONA Collective Agreement.
