1.0 FTE NURSE PRACTITIONER
37.5 hours/week, full-time with benefits
The Nurse Practitioner provides comprehensive, client-centred primary care across the lifespan, including health promotion, disease prevention, diagnosis, treatment, and chronic disease management. Working collaboratively within an interdisciplinary team, the Nurse Practitioner delivers evidence-informed care that supports improved health outcomes and enhances access to high-quality primary care services. Primary location is West Lambton CHC with outreach work integrated into the role.
Qualifications:
- Undergraduate degree in nursing from a recognized university (or equivalent combination of education and experience).
- Registration in the Extended Class with the College of Nurses of Ontario.
- Three to five years nursing experience in a community setting or combination of community and hospital or public health settings; experience in a primary care setting, an asset.
- Proficiency in the use of computers and various software applications.
- Participates in CNO Quality Assurance Program.
- Has and maintains CNO Professional Liability Insurance.
This is a full-time position, working 37.5 hours per week, Monday to Friday at our West Lambton location in Sarnia. Experience will be taken into consideration on our exciting new 6-step wage grid.
