Full Job Description
MAJOR POSITION RESPONSIBILITIES AND FUNCTIONS:

The Family Practice/Field Medicine Nurse Practitioner/Physician Assistant will provide primary care medical services in a 50% family practice and 50% mobile/field medicine capacity, in accordance with standard medical practice protocols. The provider will work under the supervision or collaboration of the appropriate Physician/Chief Medical Officer and will:
  • Provide comprehensive primary care services, including medical histories, physical examinations, diagnosis, treatment, and follow-up care.
  • Provide primary care services in both the clinic and EVCHC mobile unit, including care for unhoused individuals and underserved populations.
  • Provide patient education and individualized instruction according to treatment plans and medical protocols.
  • Prescribe appropriate medications within the provider's scope of practice and applicable medical protocols, including services such as Medication Assisted Treatment (MAT) or Prep as approved.
  • Identify physical, nutritional, lifestyle, psychosocial, and emotional concerns and refer patients to appropriate on-site or off-site resources.
  • Accurately and timely complete all required documentation in the Electronic Health Record (EHR), including diagnoses, treatment plans, orders, medications, and other required information.
  • Maintain compliance with clinical and administrative quality measures, including HEDIS metrics, patient productivity, and Quality Improvement programs.
  • Consult with the supervising/collaborating Physician or CMO for patient evaluation and treatment as needed.
  • Respond timely to pending EHR PAQ items and E-Consult specialist recommendations.
  • Provide medical services through EVCHC's telehealth platform as needed.
  • Work closely with Registered Dietitians, Health Educators, and other members of the care team to provide comprehensive patient education and care.
  • Work closely with the Clinic Manager in supporting mobile unit activities and collaborate with the CMO regarding policies and procedures related to field medicine.
  • Provide support and additional training to clinical support staff working within the mobile unit, as appropriate.
  • Participate in monthly Provider Meetings, provider recruitment activities, and initiatives that support the development of clinical team members.
  • Serve as a preceptor for Nurse Practitioner, Physician Assistant, or medical students as assigned.
  • Attend conferences, seminars, and continuing education activities as required.
  • Perform other duties and responsibilities as assigned.

POSITION REQUIREMENTS AND QUALIFICATIONS:
  • Satisfactory completion of an accredited Advanced Practice Provider training program.
  • Current California license as a Nurse Practitioner or Physician Assistant.
  • Current DEA certification.
  • Current Board Certification from an accredited organization for the applicable Advanced Practice Provider discipline.
  • Current CPR certification.
  • Minimum of two years of primary care experience; experience working in a mobile unit, community health, or with unhoused populations preferred.
  • Ability to work effectively in a small team environment and adapt to the unique needs and limited resources of a mobile unit.
  • Excellent communication, interpersonal, and teamwork skills.
  • Ability to work independently with limited supervision.
  • Understanding of the health and social needs of unhoused and underserved populations preferred.
  • Ability to work effectively with individuals from diverse cultural, ethnic, and socioeconomic backgrounds within the East Valley service area.

DOE: $155,000.00 - $183,038.00
