We are seeking a dedicated Nurse Practitioner to become part of a collaborative, multidisciplinary healthcare practice serving the Brooklyn community. This position focuses on providing high-quality adult primary care, emphasizing disease prevention, long-term health management, and patient education within a diverse patient population.

Job Function:
  • Schedule: Monday to Friday.
  • Deliver comprehensive outpatient medical care for adults, including wellness visits, health assessments, preventive screenings, and management of ongoing medical conditions.
  • Evaluate, diagnose, and treat a wide range of acute illnesses and chronic health concerns.
  • Work closely with physicians, advanced practice providers, nurses, behavioral health professionals, and other care team members to ensure coordinated treatment plans.
  • Educate patients on healthy lifestyle choices, preventive healthcare strategies, medication adherence, and self-management of chronic diseases.
  • Accurately document patient encounters, treatment plans, and clinical outcomes within the electronic medical record system.
  • Support organizational quality initiatives and actively participate in clinical meetings and performance improvement activities.
  • Perform other responsibilities and special projects as assigned by leadership.
