Family Nurse Practitioner Enhanced Primary Care

Salary ranges from $120,000.00 to $160,000.00 per year based on experience and education.

Position Summary

The Article 28 Primary Care Clinic is expanding its services and seeking a qualified, compassionate Family Nurse Practitioner (FNP) to join our interdisciplinary healthcare team. The ideal candidate will be committed to improving access to high-quality, person-centered healthcare for individuals with intellectual and developmental disabilities (IDD). This role provides the opportunity to deliver holistic primary care within a clinic designed to meet the unique needs of this specialized population.

Principal Duties and Responsibilities

  • Provide comprehensive primary and preventive care across the lifespan within an Article 28 framework.
  • Conduct assessments, diagnose conditions, and develop evidence-based treatment plans.
  • Collaborate with physicians, providers, and interdisciplinary teams to coordinate individualized care.
  • Support health promotion, patient education, and chronic disease management tailored to individuals with IDD.
  • Participate in quality improvement initiatives and contribute to the enhancement of clinic workflows and clinical protocols, with a focus on health equity and reducing barriers to care.
  • Provide clinical leadership and mentorship to nursing and support staff, fostering a culture of compassion, compliance, and excellence.
  • Maintain clinical documentation in accordance with regulatory standards and organizational policies.
  • Deliver services both in-person and through telehealth as appropriate.
