Description

Family Nurse Practitioner (FNP) to join our integrated primary team serving the West Valley Phoenix metropolitan area. This role provides the opportunity to deliver high-quality, person-centered care in an integrated, behavioral health setting. The ideal candidate is dedicated to improving health outcomes for our diverse member population and thrives in a collaborative, multidisciplinary environment.

What You'll Do
  • Provide comprehensive primary care, including assessment, diagnosis, and treatment of members across the lifespan.
  • Manage chronic and acute conditions in collaboration with behavioral health providers, nursing staff, and case managers.
  • Complete evaluations, medication management, and follow-up care in accordance with best practice standards.
  • Maintain accurate and timely documentation in the electronic health record.
  • Participate in integrated care team meetings, quality initiatives, and performance improvement projects.
  • Promote whole-person wellness through education, prevention, and culturally responsive care.


What We're Looking For
  • Current and valid FNP license in the State of Arizona.
  • National certification as a Family Nurse Practitioner.
  • DEA license (or ability to obtain upon hire).
  • Experience in outpatient or primary care settings preferred; behavioral health experience a plus.
  • Strong communication skills and a collaborative approach to care.
