Job Summary and Responsibilities

As a Nurse Practitioner, the incumbent delivers services in a team based, patient-centered primary care clinic serving insured and uninsured children and adults with a special emphasis on care for persons who experience barriers to accessing care due to income, language, cultural and social barriers. The incumbent must be willing to work a flexible schedule when circumstances necessitate as well as participating in On-Call rotation with other clinic providers. The incumbent will report to the Director Rural Clinics.

  • Performs routine physical/psychosocial assessment to collect basic data: follows the written plan of care, standards, practice guidelines, utilizes pathways, which includes notifying physicians as appropriate
  • Formulates a plan of care for individual needs, including patient population needs. Participates in the planning of routine transitional health care needs (i.e. treatment options, patient placement options)
  • Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content. Integrates education during the delivery of patient care
  • Ensures efficiency in the delivery of care to determine immediate priorities and to offer solutions for problems
  • Makes independent clinical decisions based on knowledge drawn from education and experience
  • Actively advocates for patient rights and identifies potential conflict. Seeks counsel for resolution of conflict
  • Provide team-based integrated and culturally respectful patient-centered care for a panel of patients consistent withmedical best practices and the policies and procedures of the organization.
  • Perform patient history taking, physical examination, diagnosis, develop and implement treatment plans and performrequired procedures within scope of licensure, approved credentials and clinical best practice standards.
  • Order diagnostic testing based on clinical findings and clinical best practice standards. Interpret results of diagnostictesting and implement and/or assure appropriate follow-up with the patient/family.
  • Complete appropriate coding and update EMR documentation for each patient encounter consistent with clinic procedure.Record physical findings, and formulates plan and prognosis, based on patient's condition.
  • Discuss case with physician and other health professionals to prepare comprehensive patient care plan. Submit healthcare plan and goals of individual patients for periodic review and evaluation by Medical Director. Prescribe new andmedication refills or other forms of treatment such as physical therapy, inhalation therapy, or related therapeuticprocedures
  • Provide care and education focused on individual and family engagement and behavioral change, treatment complianceand health risk factor reduction. Provide age appropriate anticipatory guidance and counseling, wellness promotion andhealth education to clients and/or families, including: nutrition and exercise, health maintenance, safety, substanceabuse (including use of alcohol, tobacco, and drugs), emotional well-being, school and activities, behavior and development

  • Nurse Practitioner - AANPCP, upon hire
  • Registered Nurse: CA, upon hire
  • Drug Enforcement Administration-Control Sub Reg Cert: CA, upon hire
  • Basic Life Support - CPR, upon hire

Preferred

  • Masters Of Nursing Nurse Practitioner and 1-3 years Minimum of one (1) year of experience as a Nurse Practitioner in an outpatient primary care clinic, community health center, etc., upon hire
